As our way of providing continued service for  your accounts maintained at our Philippine branches, you may file your request for the following services by sending us a duly accomplished request form:

  1. Bank certification
  2. Updating of passbook
  3. Replacement of lost or filled passbook

For lost passbooks, the request form must be accompanied by a duly accomplished Affidavit of Loss*. The affidavit must be authenticated by the Philippine Embassy/Consulate where you are located.

You may have the requested document picked up at your maintaining branch in the Philippines after seven (7) banking days (an Authorization to Pick-Up* must be attached along with the request form). You may also opt to have it mailed to your address outside the Philippines (subject to applicable mailing charges to be debited from your designated account).

Enclose all the required form/s in an envelope. Please indicate the required details on top of the envelope using our pre-formatted envelope design above then submit to the nearest PNB overseas office in your area.
