o	Photocopy of valid ID (SSS, TIN ID, Passport, Driver's License)	o	Photocopy of valid ID (SSS, TIN ID, Passport, Driver's License)
o	1 pc. 2x2 ID picture (white background)	o	1 pc. 2x2 ID picture (white background)
o	Broker License ID	o	Company ID
o	Certificate of Registration copy	o	Certificate of Employment copy (only for LTGC employees, retirees, and PNB Congenerics)
