PNB Credit Card Requirements

PNB Credit Card applicant must:

Complete the PNB Credit Card Application Form and signed by the client
Be at least 21 to 70 years of age
Have a contact phone number (landline) in both home and business addresses
Have at least a Gross Annual Income of P180,000.
For foreigners, in addition to the required income documents, he must attach a photocopy of Certificate of Registration (ACR) or work permit and visa.

Document Requirements:

For EMPLOYED APPLICANTS, submit the following documents:

Photocopy of the lastest Income Tax Return (ITR) and W-2
Photocopy of the last 2 months Credit Card statement (of the same credit card)
Original Pay Slip for 1 month (with the company’s name and company I.D./employee number
Original Certificat of Employment (COE)

For SELF EMPLOYED, submit the following documents:

Photocopy of the lastest Income Tax Return (ITR) and W-2
Photocopy of the last 2 months Credit Card statement (of the same credit card
Photocopy of Certificate of Business Registration together with a photocopy of Audited Financial Statements
Photocopy of last 3 months Bank Statement and Original Bank Certificate.

For BOTH employed or self-employed, attach a clear and reliable photocopy of any of the following:
